• Address Change Request - A member uses this form to change their name and/or home address with WCERS. This request cannot be processed without required supporting documentation and a signature.

• Direct Deposit Application/Change Request - A member uses this form to start, stop and change their direct deposit accounts with WCERS. This request cannot be processed without required supporting documentation and a signature.

• Sample DRO Plans 1, 2, & 3 - This form is used as a sample in the event of divorce after retirement for a member of plan 1, 2, or 3.

• Sample DRO Plan 4 - This form is used as a sample in the event of divorce after retirement for a member of plan 4.

• Sample QDRO Deferred Compensation - This form is used as a sample in the event of divorce for a member of deferred compensation. 

• Federal Tax Withholding Form (W4-P) - Please complete this form if you are requesting a change to your monthly federal income tax deductions

• State of Michigan Tax Withholding Form (MI W4-P) - Please complete this form if you are requesting a change to your monthly state of Michigan income tax deductions.

• Beneficiary Designation Defined Benefit Plan Form - This form is used to designate a retirees last monies to a beneficiary

• Email Authorization Form  - A member uses this form to request that their direct deposit statements be sent to their email address.
